Feedback forms are great.
We had them on the first YAPC.
They are still in my room in a box. Unprocessed.

A couple of questions:

1) Will we advertise the results publicly or just tell the speakers in private?
2) Will we exclude speakers from the next conference who get low results?
3) Who will put together the questions?
4) Will it be on-line or paper?
    If paper, who will type them in?
    If on-line who will code it?
